In order to ensure your health, safety & wellbeing as a member of the Sault Sabercats Team you must complete the following before the beginning of the season. We ask to avoid any confusion you complete these requirements before or during training camp:
As per the OVFL regulations and insurance policy, new athletes and returning players must undergo a health assessment prior to the season/training camp. This assessment will provide medical staff with a baseline health status of the athlete to ensure the safety of all participants. If the athlete is not deemed “fit to play” then they may be required to see the team physician or a specialist prior to playing. Health assessments can be arranged starting March 2010 by contacting the sabercats medical office at 253-3301 ext. 501 or by booking via email medical@saultsabercats.com. Athletes returning from university are asked to contact the medical team office upon return to schedule their appointment. The process takes approximately 30-45 minutes, and you are asked to bring your current health card, as well as any medications you may be currently taking. Please bring all forms in this package complete as indicated on page 1 based on your player status! to your appointment. There is a $20 charge for the medical clearance appointment. Please note that medical clearances are done only through the team medical staff to ensure continuity of care throughout the season. Medical clearances by other care providers are not accepted as clearance to play with the sabercats football club. |
